How much does surrogacy cost?
The short answer
Surrogacy typically costs $130,000 to $250,000 or more in the USA. SMARTPath’s Mexico programs frequently start around $59,000 to $75,000. Every journey has three cost groups: agency fees, surrogate compensation and medical costs. Ukraine is quoted individually.

The three parts of every budget
- Agency fees: what SMARTPath handles directly, including screening, matching, legal coordination and support.
- Surrogate compensation: base compensation, cycle fees and reimbursements, fully detailed in your contract.
- Medical costs: always billed directly by your fertility clinic in US journeys, and never hidden in a lump sum.
What “all-in” usually leaves out
When US agencies say “all-in”, they almost always mean agency services. These costs usually come from your clinic, and can add $30,000 to $60,000 or more:
- IVF cycles and procedures
- Egg retrieval or donor eggs
- Lab testing, including PGT
- Embryo storage
- IVF and transfer medications
- Embryo transfer procedures
- Costs of switching clinics mid-journey
Hidden costs to ask about
- Surrogate costs for each embryo transfer, such as travel, lost wages and childcare
- Screening and legal fees if you need to rematch
- Pregnancy complications outside insurance coverage
- Biotransport, extra testing and passport or citizenship steps in international journeys
Questions to ask any agency
- Does your price include clinic costs? Which ones?
- Does “unlimited transfers” mean clinic costs or only your coordination?
- What happens to my money if a transfer fails?
- Do refunds cover medical costs or only unused agency fees?
- Is my money held in licensed, independent escrow?
